The Case Manager is responsible for the delivery of care coordination services to a population with history of substance use and serves as a member of the interdisciplinary treatment team.

In this role, you will:

• Provide clinical case-management to clients who reside in the residence.
• Support to enhance clients’ capacity to benefit from treatment regimen.
• Crisis intervention to assist clients in dealing with sanctions, barriers to employment and self-sufficiency.
• Engage in community outreach to locate appropriate clients & assist through enrollment process.
• Assess and support services to facilitate access to specialized services.
• Assist in facilitating access to job training, job search activities, placement and stabilization.
• Advocacy and report of client activity to community based organizations and clients’ referral / mandating sources.
• Adhere to agency protocols for maintenance and security of agency equipment.
• Participate in weekly case conference.
• Complete client-centered treatment plans.
• Meet with each assigned client weekly to identify progress towards case management goals.
• Provide transportation to required appointments when necessary.
• Observe medication self-administration under supervision.

We require:

• CASAC-T; CASAC, or higher preferred.
• Minimum of 1 year of experience working with clients with substance abuse disorders and with adolescents with co-occurring and mental health problems.
• Minimum of 1 year of experience working in in an OASAS funded facility a plus.
• Commitment to providing quality services and outcome driven performance measures.
• Excellent organizational management skills with the ability to coach staff, manage, and develop high-performance clinicians, set and achieve program objectives.
• Strong networking and public relations skills with the ability to engage a wide range of stakeholders, populations, and cultures.
• Action-oriented, adaptable, and innovative approach to program planning.
• Ability to work effectively in collaboration with diverse groups of people.
