Job DescriptionJob DescriptionSalary: JOB DESCRIPTION:Case Manager, Chemical Dependency Counselor Assistant (CDCA), will perform treatment planning, assessment, crisis intervention, individual and group counseling, case management, and education services as they relate to abuse of or dependency on alcohol and other drugs under the Program Director or designee. They will serve as an advocate and liaison for and on behalf of residents rights and benefits. They assist in helping each resident navigate treatment and support services, acting as an advocate and coordinating care.PRINCIPAL DUTIES AND RESPONSIBILITIES:Orients patients to the treatment programs operation and procedures.Conducts alcohol and drug abuse counseling sessions on a regular basis per state regulation, or as part of the individual treatment plan.Conduct weekly assessment of individual client needs and assist with a plan to address those needs.Conducts intake interviews with clients to gather information about their substance abuse history, mental health status, and other relevant factors.Based on the information gathered during assessments, they will help to develop treatment plans with clients that will address their unique needs and goals.Develops within (10) days of entry or as required by state guidelines, whichever is more stringent, a formal treatment plan documenting patient goals and update utilizing the same guidelines.Implements or revises patient treatment plans with the patient as required per state regulation and record a review of the patients progress.Monitor clients' progress throughout their treatment and make adjustments to their treatment plans as needed.Provide counseling services to clients, either one-on-one or in group settings. They may help clients to identify triggers for substance use, develop coping skills, and work through emotional issues related to their addiction.Participates in the Treatment team and documentation in EMR.Assist patients in identifying, maximizing, and relating their strengths to appropriate social, educational (academic and vocational), and occupational goals.Help clients to access social services, such as housing or employment assistance, that can support their recovery.Helps patients prepare and adjust to a drug free existence, and to offer assistance (emotional) in problematic issues.Maintains patient records that are complete, comprehensive and well organized. Assure documentation is completed per OMHAS, regulatory, and agency requirements following best practices.Maintains a case load of patients, meeting all requirements for sessions and documentation.Attend required meetings, including individual and group supervision, treatment team meetings, training, and community events as appropriate. Participate in at least one hour of supervision per week.Other duties may be assigned.Evaluate, facilitate and monitor services and clients progress in relationship to established goals and objectives.Work to obtain aftercare placements such as sober living, partial hospitalization, or outpatient placement for aftercare services.Reviews and coordinates with the team and provider on decisions involving hospitalization, respite and transfers/level of-care changes. Implement plan of action. Maintains ongoing dialogue with intake regarding status changes.Facilitate community resource availability based on client needs (e.g., day program, guardians) to ensure maximum social integration.Complete thirty, ninety, and annual follow up calls.Facilitates client benefits (e.g., SSI, Medicaid, SSB, Medicare).Holds a basic understanding of alcohol/drug abuse and addiction. Additionally, keep updated on current drug abuse and treatment information, especially current trends in Opioid Maintenance Treatment.Ensure compliance with the Ohio state regulations and ethical guidelines for CDCA practice.Other duties may be assigned.QUALIFICATIONS:Must have High School Diploma or GED.CDCA certification or higher through the Ohio CDP Board is required.An associates degree in human services, Chemical Dependency, or another related field highly preferred.CPR/BLS Certification required.1-2 years experience working with substance use preferred.REQUIRED SKILLS OR ABILITIES:Required Skills for this position include but are not limited to:Must demonstrate a current knowledge of counseling and research findings and literature pertaining to alcohol/drug abuse, domestic violence, sexual assault, child abuse/neglect, trauma informed care and mental health issues.Ability to interact with a multi-disciplinary team to assist in facets of the recovery program relating to physical health.Have the ability to communicate effectively with all members of the treatment team to ensure the safety of the client.Flexibility to adapt to schedule changes and assumption of responsibilities not delineated in the job description which is related to work as a member of an addiction treatment team.Demonstrated knowledge of community resources and referral sources.Attention to detail and organization.Computer literate with experience in a Windows environment.Ability to work independently with minimal supervision.Ability to prioritize work and work-related responsibilities.Excellent observation skillsCustomer/Client FocusStress Management/ComposureEmpathy and Compassion:Ability to connect with clients and understand their experiences.Advocacy Skills: Ability to advocate for clients' rights and needs.Problem-Solving Skills:Ability to identify and address challenges that clients face.Cultural Competency:Understanding and sensitivity to diverse cultures and backgrounds.Knowledge of SUD and Recovery: Familiarity with SUD, treatment options, and recovery principles.SUPERVISORY RESPONSIBILITY:There is no supervisory responsibility in this role.
