Case Manager for Developmental Disabilities (Hybrid Schedule)Join a dynamic Supported Living agency and work with a team of supportive hardworking Case Managers.The Case Manager is responsible for the coordination of supported living services for adults with developmental disabilities with a maximum caseload of 10 individuals. The Case Manager coordinates and assigns schedules for support staff live-in staff and IHSS staff.In coordination with the Director and Associate Director the Case Manager will supervise train monitor and evaluate all paid support staff. It is critical that the Case Manager understand model maintain and articulate the philosophies values and goals of supported living as they relate to each individual and their lifestyle.The Case Manager will be part of a team that includes the individual receiving services service professionals family members and others identified in the Individual Support Plan (ISP).Key responsibilities include:- Attending quarterly meetings with SD Regional Center the client and the circle of support.- Helping remedy issues with MediCal CalFresh and other programs.- Reviewing staff time-off requests and arranging for coverage.- Using scheduling software to input schedules and review timesheets each pay period for accuracy.- Visiting clients and staff on a regular basis.- Responding to texts and emails in a timely manner and working occasional shifts as needed.Excellent written and verbal communication skills are a must.Job Type: Full-time Hybrid (3 days in office 10:00 am-3:30 pm 2 days work from home)Compensation and Benefits:- Salary: $29.00-$32.00/hour DOE- Health Insurance- 2 weeks paid time off- 401K retirement plan after 1 year (4% match)- Mileage reimbursement- Phone reimbursementWe look forward to hearing from qualified candidates.Laura BrownHR ManagerRequired Experience:Manager Key Skills Project Portfolio Management,Motivational Interviewing,HIPAA,Computer Skills,Intake Experience,Lean Six Sigma,Conflict Management,Case Management,Team Management,Program Development,Social Work,Addiction Counseling Employment Type : Full-Time Experience: years Vacancy: 1 Hourly Salary Salary: 29 - 32
