Job DescriptionBe a part of a world-class academic healthcare system at UChicago Medicine as an Adult Inpatient Case Manager at our main medical this role you will partner with the rest of the clinical team to provide patients with the highest quality of care. Our Care Coordination team delivers the highest quality of care while ensuring continuation of care for our patients is a seamless process.The Case Manager is a member of the leadership team charged with developing and maintaining patient care standards. They will coordinate and oversee the patients care primarily during the inpatient phase of treatment. The Case Manager will collaborate with other members of the care team and multidisciplinary team as dictated by patient need and appropriate utilization of resources.Essential Job Functions:Plans monitors and coordinates patient care using the plan of care or clinical pathway.Identifies patient and family needs for consultation with other multidisciplinary specialists and assures needs are met.Plans and conducts or participates in patient care conferences.Assesses patient discharge needs. Assures completion of all discharge related functions on a timely basis.Provides consultation to staff nurses in evaluating current patient status and plan for care.Assures that comprehensive information necessary for the documentation of the patients hospital course is available to all care team members. Documents in the patient record as appropriate.Performs other duties as assigned.Required Qualifications:A bachelors degree in nursing from an NLN accredited school of nursingA current Illinois Registered Nurse license3 or more years of Nursing experienceRequired Education: Associate Degree in Nursing; BSN degree or higher preferred. Associate degree Nurses are required to enroll in a BSN program within six (6) to nine (9) months of hire and make progress toward and complete their BSN degree within 30 months. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations such as internal equity.
