Hello, we're Hampton-Newport News Community Services BoardSince 1971, Hampton-Newport News Community Services Board has provided a comprehensive continuum of high-quality, essential, and community-based services to citizens of the Peninsula who are living with a mental illness, developmental disabilities, and substance use disorders.The Service Coordination Team provides crisis support services to individuals who could benefit from more intensive outpatient services in the community. The team provides ongoing monitoring of safety, coping skills education, recovery tools, and linkages to CSB and community resources. One of the team's goals is to assist individuals with maintaining stabilization in the community upon discharge from a psychiatric facility. This team assists individuals enrolled in the CSB system and is an excellent support to the Reinvestment Case Management Team, Emergency Services, and other CSB programs. The team utilizes Case Management and Peer Support Staff. The Peer Support Specialist is a vital component of the team- sharing real-life recovery experiences which benefit the individuals served on their recovery journey.What it's like to be a Case ManagerAs a Case Manager on our Service Coordination Team at Hampton-Newport News Community Services Board, you will provide intensive support services to individuals recently discharged from the hospital or those at risk of hospitalization. Your expertise will be vital in linking clients to HNNCSB and community resources, navigating both physical and mental health needs, and teaching essential coping skills. You will ensure continuity of services for mental health, developmental disabilities, and substance use disorder populations, assisting clients with their basic physical, financial, social, and personal needs. Your role encompasses outreach, psychological assessments, treatment planning, and advocacy, along with crisis intervention and medication supervision.You'll meet with clients, families, and care providers to assess needs, prepare mental health status summaries, and respond to inquiries about available services. By actively participating in discharge planning and monitoring client progress, you'll play a crucial role in helping clients achieve their goals and improve their overall well-being.Are you the Case Manager we're looking for?To excel as a Case Manager on the Service Coordination Team at Hampton-Newport News Community Services Board, candidates must possess a Bachelor's degree in Human Services, such as Social Work, Psychology, Sociology, or Counseling, along with a year of relevant case management experience. Strong communication and interpersonal skills are essential for building rapport with clients, families, and care providers. A valid Virginia Driver's License is necessary for facilitating client visits and appointments.Proficiency with case management software and tools will be critical in documenting assessments and monitoring client progress effectively. Additionally, candidates should demonstrate empathy, integrity, and the ability to advocate for clients' needs within the healthcare system. Excellent organizational skills will ensure efficient management of appointments and client interactions, while a solid understanding of mental health, developmental disabilities, and substance use disorders will enhance service delivery and support for the populations we serve.Knowledge and skills required for the position are: Bachelor's degree in Human Services, such as Social Work, Psychology, Sociology, or Counseling.One (1) year of experience in case management services.
