Job DescriptionJob DescriptionProvides respite, advocacy, training of developmental life assets, and mentoring services to children/adolescents in Hillsborough County. Participates as a member of a multidisciplinary treatment team. Reports to the unit/department supervisor. Ability to read, analyze, and coordinate with multidisciplinary teams regarding procedures, and governmental regulations. Ability to write reports and document mentoring services and effectively communicate verbally with, provide information to, and respond to questions from managers, patients, service providers, managed care entities, family members, and other stakeholders. PHYSICAL DEMANDS:Abilities in reaching, bending, talking, fingering, sitting, carrying, standing, grasping, fine hand coordination, ability to hear, read, write, and the ability to remain calm under stress. Ability to reach with hands and arms, climb stairs, balance, stoop, kneel, crouch, or crawl. Specific vision abilities include close, distance, color, peripheral, depth perception, and the ability to adjust focus. Must be capable of lifting 50 pounds of dead weight alone. May be required to complete standing presentations. WORK ENVIRONMENT:High-paced, collaborative team performance with an emphasis on critical thinking and decision-making with guidance from program supervisor/manager. Climate-controlled office environments with frequent exposure to clients. Some programs may require traveling within Hillsborough County via personal vehicle or agency vehicle to facilitate in-home client services in a variety of socioeconomic neighborhoods. FUNCTIONAL RESPONSIBILITIES INCLUDE:Provide therapeutic mentoring/respite services in a timely and reliable manner under the policies, procedures, and applicable provisional standards.Complete all documentation based on established timelines as per program requirements.Provides responsible activities with consistent supervision to clients and their families. Keep the direct supervisor and treatment team informed at all times of relevant consumer and family issues. Maintain efficient workflow management and organization as per program requirements.Meets with case manager and family to establish specific details for implementing home and community-based rehabilitative and respite services.Other duties as assigned or needed as per program requirementsMon-Fri 8:00AM-4:30 PMFull-Time Employment
