We are seeking a caring and dependable Certified Nursing Assistant (CNA) to join our interdisciplinary team and provide personal hands-on support to hospice patients in their homes.What Youll DoAs a hospice CNA youll play a vital role in helping patients and families through meaningful support including:Providing personal care based on the individualized care plan (bathing grooming feeding dressing toileting etc.)Maintaining patient comfort and safety in their home environmentMonitoring and reporting changes in the patients condition to the RN Case ManagerAssisting with transfers mobility and range-of-motion exercisesEducating and supporting families on patient care techniquesDocumenting clinical notes accurately and on timeParticipating in team meetings and quality improvement initiativesQualificationsActive CNA certification in the state of employmentGraduate of an approved Certified Nursing Assistant programMinimum 1 year of nursing assistant experience (hospice home health or community-based preferred)Current BLS/CPR certificationValid drivers license reliable transportation and car insuranceAbility to work independently and compassionately in home settingsExcellent communication professionalism and time management skillsPlease note: Legacy Hospice is a drug-free workplace.
