Position Overview (Charge Nurse – RN/LPN)

Polaris Rehabilitation & Care Center is seeking an experienced and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to serve as a Charge Nurse. This leadership role is responsible for coordinating the daily nursing operations during an assigned shift while ensuring exceptional resident care, regulatory compliance, and clinical support for the nursing team.

As the Charge Nurse, you will provide direct resident care while serving as the clinical leader for your shift. You will supervise nursing staff, coordinate resident care, respond to changes in resident condition, and promote a culture of teamwork, accountability, and clinical excellence. This position is ideal for a nurse who enjoys mentoring others while remaining actively involved in resident care.

Key Responsibilities

  • Lead and coordinate nursing operations during the assigned shift

  • Provide direct nursing care in accordance with individualized care plans, physician orders, and facility standards

  • Monitor resident conditions, assess changes in health status, and ensure timely clinical interventions

  • Administer medications, treatments, and therapeutic procedures within scope of practice

  • Supervise, delegate, and support Certified Nursing Assistants (CNAs), Medication Aides, and nursing staff during the shift

  • Serve as a clinical resource by providing coaching, guidance, and real-time problem solving for nursing staff

  • Ensure accurate, timely, and compliant clinical documentation within the electronic medical record (EMR)

  • Communicate changes in resident condition promptly to physicians, families, and interdisciplinary team members

  • Assist with admissions, transfers, discharges, and continuity of resident care

  • Promote compliance with CMS regulations, Wyoming state regulations, infection prevention standards, and facility policies

  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives and resident safety programs

  • Respond promptly to emergencies, incidents, and changes in resident condition using sound clinical judgment

  • Foster a positive, collaborative, and resident-centered work environment

Qualifications

  • Graduate of an accredited nursing program

  • Current, active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing in the State of Wyoming

  • Current CPR or Basic Life Support (BLS) certification

  • Previous experience in skilled nursing, rehabilitation, long-term care, or post-acute care preferred

  • Previous Charge Nurse or supervisory experience preferred

  • Strong knowledge of CMS regulations, infection prevention, nursing documentation, and long-term care standards

  • Demonstrated leadership, organizational, and communication skills

  • Excellent clinical assessment, critical thinking, and decision-making abilities

  • Ability to manage multiple priorities in a fast-paced healthcare environment

  • Commitment to delivering compassionate, resident-centered care while supporting and developing nursing staff
