This is hands-on impactful work that supports families in their homes communities schools and daily environments.Please note: You do NOT need a clinical license to apply.If you meet the education and experience requirements you can excel in this role.What You Will DoThis position offers the opportunity to support youth and families in meaningful and practical ways. Each day brings new opportunities to create positive change.Provide Support in Homes and Community SettingsDeliver family-focused therapeutic interventions tailored to youth with significant emotional or behavioral needs.Support members in managing daily challenges and building healthy coping skills.Crisis Support and PlanningAssist in developing individualized crisis plans.Respond professionally and calmly during crisis situations to ensure safety and stability.Treatment Planning and DocumentationHelp create and revise Person-Centered Plans (PCPs) and related clinical tools.Maintain accurate timely documentation in alignment with regulatory and agency standards.Care CoordinationCoordinate assessments and ensure linkage to needed medical psychological and community services.Advocate for members across multiple systems ensuring their needs are met.Community and Natural Supports EngagementIdentify and engage community resources and natural supports to promote long-term stability.Team Collaboration and Professional DevelopmentParticipate in weekly clinical meetings and case consultations.Attend required clinical supervision and ongoing training to strengthen skills.Confidentiality and Cultural SensitivityUphold HIPAA and confidentiality requirements.Provide culturally competent services that respect each familys values and needs.Transitional and Discharge SupportAssist in planning for transitions and discharges with appropriate follow-up support.Who Thrives in This RoleSuccessful candidates are:Empathetic and patientOrganized and reliableStrong communicatorsSkilled at building rapport with youth and familiesComfortable working independently and collaborativelyFlexible and able to adapt to changing environmentsQualificationsCandidates must meet one of the following educational and experience combinations:Bachelors degree in a human services field with at least 2 years of experience working with the population servedBachelors degree in any field with at least 4 years of experience working with the population servedMasters degree in a human services field with at least 1 year of experienceMasters degree in any field with at least 2 years of experienceNo clinical license required.Working ConditionsThis is a community-based role.
