We are seeking a skilled Chronic Care Management Coordinator to support our clinical team and help deliver exceptional care to patients with chronic conditions.Position SummaryThe Chronic Care Management (CCM) Coordinator is responsible for coordinating and delivering high-quality chronic care and/or principal care management services to eligible patients. This role works closely with providers, clinical staff, and patients to ensure timely follow-up, effective care planning, and proactive management of chronic conditions.Key ResponsibilitiesManage CCM/PCM patient caseloads, ensuring compliance with program requirements and documentation standardsConduct monthly CCM outreach, health assessments, and follow-up callsDevelop, update, and maintain individualized care plansCollaborate with providers, RNs, LPNs, and support staff to coordinate patient careTrack and document all patient interactions in accordance with CMS guidelinesMonitor patient progress, identify needs, and facilitate referrals when appropriateAssist with care transitions and patient education related to chronic disease managementSupport quality improvement initiatives within the practiceBenefitsCompetitive compensation Health, dental, and vision insurancePaid time off and holidaysSupportive and collaborative work environmentOpportunities for professional growthLocationTrident Pain Center, LLCEmployment TypeFull-Time Requirements: RequirementsRN or LPN license (active and in good standing)Experience with Chronic Care Management (CCM) and/or Principal Care Management (PCM) requiredStrong communication and patient-engagement skillsAbility to work independently, stay organized, and manage a high-volume caseloadStrong clinical judgment and attention to detailProficiency with computers and electronic health recordsPreferred QualificationsExperience with eClinicalWorks (eCW)Experience in Pain Management or related specialtyKnowledge of Medicare CCM/PCM billing and compliance standardsCompensation details: 25-35 Hourly WagePI9a318b5-
