Job Title:                              Clinical Coordinator

Job Status:                          Full-time, Exempt

Report To:                           VP of Operations


JOB SUMMARY

The Clinical Coordinator provides clinical oversight and coordination for our elders across Vista Living Care's Memory Care and Home Care services. This position focuses on assessments, care planning, medication oversight, incident management, regulatory compliance, family communication, and clinical monitoring to ensure safe, high-quality, person-centered care. The Clinical Coordinator partners closely with Care Partners, Medication Assistants, healthcare providers, families, and operational leadership to identify clinical needs, implement appropriate interventions, and maintain compliance with regulatory and company standards.


ESSENTIAL DUTIES & RESPONSIBILITIES

·        Conduct elder and client assessments, including admissions, routine reassessments, and evaluations following changes in condition.

·        Develop, implement, and update individualized care plans, monitoring elder/client conditions and coordinating appropriate interventions, follow-up actions, and care plan modifications.

·        Oversee medication management processes, documentation, and regulatory compliance.

·        Coordinate care transitions, including admissions, hospitalizations, hospice enrollments, and discharges.

·        Oversee wound monitoring, treatment coordination, and clinical documentation.

·        Serve as a clinical resource for families regarding care plans, changes in condition, and clinical concerns, including participation in family conferences as needed.

·        Collaborate with physicians, hospice providers, pharmacies, and other healthcare partners to support continuity of care.

·        Provide clinical coaching, training, and competency development for Care Partners, PCAs, and MA’s in medication management, dementia care, infection prevention, and resident safety.

·        Partner with Operations and Human Resources regarding performance concerns, training needs, and corrective action plans.

·        Ensure compliance with Department of Health regulations, Board of Pharmacy requirements, company policies, and regulatory standards through audits, documentation review, and quality assurance activities.

·        Oversee incident reporting, investigation, follow-up, resolution, and documentation.

·        Identify clinical trends, risks, and quality concerns and implement corrective actions and quality improvement initiatives as appropriate.

·        Assist with on-call responsibilities as assigned, support other Vista Living Care locations as needed, and perform other duties as assigned by management.


QUALIFICATIONS

·        Licensed Practical Nurse (LPN), Registered Nurse (RN), or other qualified healthcare professional with experience in senior living, memory care, home care, hospice, or long-term care.

·        Experience with care plans, medication management, incident reporting, regulatory compliance, staff training, and family communication.

·        Strong knowledge of dementia care, person-centered care practices, and elder safety.

·        Ability to assess concerns, identify risks, coordinate appropriate follow-up, and collaborate with healthcare providers and operational leadership.

·        Strong organizational, communication, and problem-solving skills.

·        Experience with electronic health records, care management systems, and Microsoft Office applications.

·        Ability to work flexible hours, including occasional evenings, weekends, and on-call responsibilities.

·        Must possess sufficient English proficiency to effectively communicate with elders, families, and team members to ensure safety, quality care, and positive engagement.

·        Ability to successfully complete a criminal background check, employment verification, Motor Vehicle Report, and NMDOH clearance.

·        Must have reliable transportation, a safe driving record, a valid driver’s license, and current motor vehicle insurance.


WORKING CONDITION & PHYSICAL DEMANDS

Working Conditions:

·        Work is performed in Memory Care homes, Home Care environments, and occasional community-based settings.

·        Frequent interaction with elders, families, staff, healthcare providers, and community partners.

·        Exposure to normal residential and healthcare conditions, including potential contact with bodily fluids, communicable illnesses, cleaning agents, and other healthcare-related hazards while following established safety and infection control procedures.

·        Requires flexibility in scheduling, including occasional evenings, weekends, holidays, on-call responsibilities, and travel between locations as needed.

·        Must be able to respond appropriately to emergencies, incidents, changes in condition, and operational needs.


Physical/Cognitive Demands:

·        Frequent standing, walking, bending, reaching, and movement throughout care environments.

·        Occasional lifting, carrying, pushing, or pulling of up to 40 pounds.

·        Must be capable of demonstrating care techniques, observing care delivery, and providing hands-on support during training, emergencies, staffing needs, or quality assurance activities.

·        Requires manual dexterity, visual acuity, and computer proficiency to perform assessments, review documentation, utilize electronic systems, and complete job-related tasks.

·        Must be able to communicate effectively, observe changes in condition, exercise sound judgment, and respond appropriately to safety concerns and emergencies.

·        Ability to prioritize tasks, solve problems, and manage multiple responsibilities in a fast-paced environment.
