The ideal candidate is adaptable clinically strong and comfortable working across multiple care settings within an integrated health system.Key ResponsibilitiesProvide clinical pharmacy services across inpatient acute-care units and ambulatory clinicsPerform medication order verification therapeutic monitoring and clinical reviewSupport anticoagulation clinics through dosing monitoring and patient follow-upAssist oncology services with medication coordination and supportive care reviewParticipate in interdisciplinary rounds and collaborate with providers on drug therapy plansConduct medication reconciliation and support transitions-of-care workflowsRespond to drug information inquiries from clinicians and patientsEnsure adherence to regulatory guidelines safety practices and organizational policiesDocument clinical interventions and maintain accurate pharmacy recordsUtilize electronic medical record (EMR) systems for order management and documentationQualificationsBS in Pharmacy required; PharmD preferredActive Pennsylvania Pharmacist LicenseRecent hospital or acute-care pharmacy experience preferredExperience in ambulatory practice anticoagulation or oncology support strongly preferredProficiency with EMR systemsStrong communication and clinical problem-solving skillsWork Environment & ScheduleThis is a 1213 week assignment based in central Pennsylvania within a large multi-site healthcare system. Full-time schedule; specific shift details provided during the interview.
