Educates and acts as a consultant to patients/family and members of the health care team related to optimal drug therapy and participates in process improvement activities and drug use evaluations. Works with multiple medication automation systems.Interprets and screens medication orders for appropriate agent dose route allergies and potential drug interactions. Safely compounds and dispenses medications for administration to patients.Evaluates patients orders appropriate laboratory tests and interprets serum drug levels utilizing pharmacokinetic principles; provides prescribers with dosage regimen recommendations to optimize therapeutic outcomes reduce patient risk and monitor the costs associated with drug therapy.Provides appropriate supervision and direction within appropriate skill level to technicians residents interns and other support personnel.Engages in continuous study of the entire professional field to maintain the professional competence knowledge and skills necessary for satisfactory performance of all assigned responsibilities. Completes required continuous training and education including department and Spectrum Health specific requirements on a timely basis.Provides education and counseling to patients/family/significant others/members of the health care team in order to optimize outcomes and appropriate use of medications.Ensures that services are provided in accordance with State and Federal regulations organizational policy and accreditation/compliance requirements.QualificationsRequired Bachelors Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy2 years of relevant experience hospital pharmacy PreferredAmerican Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice PreferredLIC-Clinical Pharmacist - STATEMI State of Michigan 90 Days requiredLIC-Pharmacist Controlled Substance - STATEMI State of Michigan 90 Days requiredCRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 180 Days required OrCRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 180 Days required OrCRT-Basic Life Support (BLS) - AHA American Heart Association 180 Days required OrCRT-Basic Life Support (BLS) - ARC American Red Cross 180 Days requiredIdeal candidate will possess a PharmD Degree and Inpatient Pharmacy experiencePGY1 or PGY2 Residencies would be a great plusHow Corewell Health cares for youComprehensive benefits package to meet your financial health and work/life balance goals.
