Job DescriptionJob DescriptionJOB SUMMARYThe Qualified Developmental Disability Professional / Quality Assurance shall perform in accordance with ICF/DD Title XIX regulations guidelines. All functions shall be performed in accordance with the established policies and practices of the Federal, State and local policies that govern the Title XIX funding source. ESSENTIAL DUTIES AND RESPONSIBILITIES:Assess and monitor the health and well-being of individuals servedConsult with medical professionals as needed regarding care or treatmentObserve staff performance and interaction with individualsReview medication administration for accuracy and complianceMonitor employee performance related to health and safety standardsDevelop and implement quality assessments to ensure effective servicesEnsure all evaluations and follow-ups are completed in a timely mannerCoordinate and track treatment recommendations across providersMonitor school and day program treatment plans for alignment and progressOversee and ensure proper management of consumer fund accountsAdvocate for individuals served to ensure quality outcomes are achievedPerform other related duties as assignedGroup Home Coordinates, integrates, and monitors each resident's care and active treatment. Advocate, and supervise to ensure residents receive the services and support they need to develop skills and live as independently as possible. Provide quality management support and supervision to assigned house managers.Ensures that residential homes are in compliance with safety and building codes and licensing standards.Provides twenty-four hours on call coverage as needed and as assigned by Assistant or Program Director. Participate in the annual state survey. Participate in CARF accreditation. Participates in the admission and discharge of individuals served.Conducts investigations and prepares reports of findings to appropriate Volunteers of America Southeast Louisiana management.Providing training to DSP and Home Managers as needed.Evacuate with Community Living Services program during evacuation.Quality Assurance Ensures quality outcomes are achieved.Collect and aggregate data quarterly for outcome management reports for all community homes under direct supervision.Collect and aggregate data for the 1st and 25th of the month reports for all community homes Collect data for driving assessments for all of CLS community homesConduct monthly audits of client records, staff documentation, medical records, and incident reports to ensure compliance with ICF standards (State and Federal regulations).Perform on-site quarterly quality assurance reviews in each ICF home to evaluate program, safety, cleanliness, staff performance, and consumer satisfaction.Verify that all required daily, weekly, and monthly entries are completed by Direct Support Professionals (DSPs), Nurses, QDDPs, Home Managers and contract services (ie dietician ect)Provide training, guidance, and technical assistance to direct support staff and home managers regarding documentation standards and best practices.Participate in survey preparation and assist during state or federal surveys.Maintain up-to-date knowledge of ICF regulations, CMS guidelines, and other governing agency requirements.Monitor compliance with residents’ Individual Program Plans (IPPs), Behavior Support Plans, and Health Care Plans.Residential Care Functions Ensure residents are treated with respect and dignityParticipate in consumer interdisciplinary team meetings, annual IHP’s and Quarterly updates Report incidents of neglect or abuse of consumersReport all critical incidents to (SIMS) the Louisiana Department of Health (DHH) within 24 hours required by DHH. Perform IDT- interdisciplinary team meeting for residents as needed.Ensure residents access the community. Ensure resident have access to medical services.Assist as support staff if needed to provide Direct Support Care. Assist residents in developing a circle of support. Therap Charting and DocumentationReview the following documentation components for accuracy and compliance:ISP Data Collection: Ensure daily goals, objectives, and outcomes are recorded and correspond with the current Individual Support Plan (ISP).Medication Administration Records (MAR): Ensure accuracy of medication entries, documentation of administration, and timely recording of PRNs and missed doses.Incident Reports (GERs): Review all General Event Reports for completion, proper follow-up, and submission within policy timelines.Behavioral Data: Confirm behavioral incidents and data are accurately logged and align with the approved Behavior Support Plan (BSP).Progress Notes: Verify that monthly summaries and progress reports are entered by responsible staff (QDDP, RN, etc.) and reflect meaningful documentation.Staff Notes and Communication Logs: Ensure all entries maintain professional tone, objectivity, and confidentiality.Health Tracking: Confirm entries for MARs, vital signs, bowel movements, seizure logs, menstrual tracking, and weights are up to date and reviewed by nursing.ADDITIONAL DUTIES AND RESPONSIBILITIES (Non-essential)Adhere to the designated dress code as stipulated for the specific role.SUPERVISORY DUTIES:NoneKEY CONTACTSDay Program VendorsLouisiana Department of HealthOffice of Citizen for Development Disabilities Medical ProvidersAll Stakeholders KNOWLEDGE, SKILLS AND ABILITIES:This position requires knowledge, skills and abilities equivalent to:Bachelor’s degree in a human services disciplineMinimum of two years of experience working with adults with mental illness or developmental disabilitiesDemonstrated supervisory experience and ability to effectively manage peopleKnowledge and understanding of Title XIX regulations related to community-based residential homesExcellent verbal and written communication skillsProficiency with personal computers is desirableMust have a valid Louisiana Drivers License and be insurable under the Volunteers of America Southeast Louisiana vehicle insurance. Applicant must be able to pass a criminal history investigation, able to obtain Cardio Pulmonary Resuscitation and First Aid certification. The position requires travel/work within all regions located in Southeast LouisianaPHYSICAL REQUIREMENTS: Physical requirements include sitting, standing, bending, pushing, pulling, squatting and lifting. The ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary. Move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment.Operating office equipment requiring continuous or repetitive hand/arm movements.
