Job DescriptionJob DescriptionWe are seeking a Community Behavioral Health Worker-ACT!Southeastern, OHJoin our Team!Integrated Services for Behavioral Health (ISBH) is a community-minded, forward-thinking behavioral health organization helping people along the road to health and well-being. We meet people in their homes and communities and help connect them to the resources. We serve Southeastern and Central Ohio with a comprehensive array of behavioral health and other services, working with local partners to promote healthy people and strong communities. Our services are intended to be collaborative and personalized for the individual.As a Community Behavioral Health Worker / Vocational Specialist on the Assertive Community Treatment (ACT) team, you will support individuals with serious mental illness in achieving recovery goals through meaningful employment. This position is essential to helping people explore, obtain, and maintain competitive, integrated employment as part of a multidisciplinary team. The role adheres to the Dartmouth Assertive Community Treatment Scale (DACTS) fidelity standards, with a strong emphasis on community-based, person-centered vocational support.The pay range for this position is $19.00 - $22.26 per hour based on experience, education, and/or licensure.Essential Functions:Assist individuals in exploring their interests and goals related to work, education, and meaningful daily roles.Provide individual vocational assessments, job development, benefits counseling, job coaching, and ongoing follow-along supports.Cultivate relationships with local employers and community partners to identify job opportunities aligned with individual strengths and preferences.Educate clients on how employment affects benefits (SSI/SSDI, Medicaid, etc.) using evidence-based tools.Provide behavioral health and case management services in homes, workplaces, and community settings.Serve a shared caseload as a full member of the ACT team; participate in daily team meetings and treatment planning.Document all services accurately and promptly in accordance with agency standards and Medicaid billing guidelines.Meet productivity and fidelity requirements as defined by ISBH and DACTS.Participate in cross-training, utilization review, and quality improvement initiatives.Support program expansion into surrounding counties as needed.Minimum Requirements:High School Diploma or GED required; Bachelor’s degree in a behavioral health-related field preferred.Minimum of three years of relevant experience (e.g., behavioral health, employment services, vocational rehabilitation).Valid driver’s license, insurance, and reliable transportation required for community-based work.Demonstrated commitment to recovery principles and cultural humility.Knowledge of local community service systems and employer networks.Comfortable working in an Internet-based, mobile office environment.Able to engage and motivate individuals with complex behavioral health needs.Strong interpersonal, verbal, and written communication skills.Enjoy a great work environment with an excellent salary, generous paid time off, and a strong benefits package.Benefits include:MedicalDentalVisionShort-term DisabilityLong-term Disability401K w/ Employer MatchEmployee Assistance Program (EAP) provides support and resources to help you and your family with a range of issues.To learn more about our organization: https://ISBH.org/OUR MISSIONDelivering exceptional care through connectionOUR VALUESDignity - We meet people where they are on their journey with respect and hopeCollaboration - We listen to understand and ask how we can best support the people and communities we serveWellbeing - We celebrate one another's strengths, and we support one another in being wellExcellence - We demand high-quality care for those we serve, and are a leader in how we care for one another as a teamInnovation - We deeply value a range of perspectives and experiences, knowing it is what inspires us to stretch past where we are and reach towards what we know is possibleWe’re an equal opportunity employer.
