Job DescriptionJob DescriptionSalary: $48,000The Community Health Worker Coordinator is the manager of social care delivered by Community Health Workers. They provide direct supervision of any Community Health Worker staff and others assigned to related programs and services. They implement policies and processes for services and programs related to Community Health Worker social care delivery. The position reports directly to the Executive Director.Roles and ResponsibilitiesSocial Care Delivery Manager (Approximately 30% of time)Manage administration of social care by CHW staff and for assigned programs and services. This includes oversight for lead generation, lead review, lead conversion, case openings, case interactions, case resource connections, and case closings.Assist in Beacon meeting all program, project, and contractual deliverables Complete required in data cleaning within SalesForce Create, refine, and update project manuals, guides, and other tools for staff on at least an annual basis. Community Health Worker Staff Supervisor (Approximately 50% of time)Engage in direct supervision of staff weeklyEngage in indirect supervision of staff weeklyLead regular meetings (Huddles and 1:1) with staffDevelop and enforce scheduling of staff to provide effective coverageProvide supervision and guidance to staff assigned to specific projectsEnsure timely response by Community Health Workers with in-person interactionsEnsure data completeness and accuracy from all service line or program staffLeadership Team Member (Approximately 20% of time)Actively participate in leadership meetings and initiativesEngage in relationship building with community partner agencies and their personnel.Other duties as assigned. Sensitive to the needs of the poor, uninsured, elderly, disabled, chronically ill, or other marginalized peopleStrong computer skills and the ability to accurately document client activities in an electronic health recordComfortable working in a busy, loud environmentWorkdays: Monday-Thursday (office) Friday (remote)
