Location:Calmont Operations BuildingDepartment:Clinical Support & PreventionShift:First Shift (United States of America)Standard Weekly Hours:40Summary:Responsible for performing a wide range of activities for Star Kids members to positively affect individual health outcomes. The CHW Service Coordinator will monitor a group of level 3 members who do not meet the requirements for an LMSW or RN named service coordinator. Activities include but are not limited to outreach assessment Member education service coordination and Member support.Additional Information:The Bilingual Community Health Worker Service Coordinator is responsible for performing a wide range of activities for Star Kids members that will positively affect individual health outcomes. The CHW Service Coordinator will monitor a group of level 3 members who do not meet the requirements for an LMSW or RN named service coordinator. Responsibilities include (but are not limited to) Outreach Assessments Member Education Service Coordination and Member Support.Required Education/Experience:High school diploma or equivalent.Community Health Worker (CHW) training and certification.Minimum of five (5) years experience in community health social services community advocacy community outreach health plan Member Services and/or Engagement or a combination of relevant experience and education.Understanding of the community in the Service Delivery Area through shared experiences and/or a strong desire to help people in vulnerable communities. Bi-lingual (English/Spanish) Required. Expert proficiency in conversational reading writing and translating medical terms as well as diagnoses. Must pass Spanish test prior to hire.Private reliable transportation.Excellent communication skills with the ability to establish and maintain positive and effective work relationships with coworkers members and providers.Demonstrated ability to meet set deadlines and manage priorities. Knowledge of basic medical terminology.Willingness to work in a frequently changing environment.Computer skills and experience with Microsoft Office products.Certification/Licensure:TX Community Health Worker certification.**Star Kids program experience is a huge plus.**About Us:Cook Childrens Health PlanCook Childrens Health Plan provides vital coverage to nearly 120000 people in low-income families who qualify for government-sponsored programs in our six county service region.
