Job Description

This Position is *IN PERSON *Monday through Fridayrequiring one weekend a month; This is a full time, 40 hour perweek position. This position is housed in an Intensive OutpatientTransitional Living Home.Purpose: The CSWprovides direct care services to clients who qualify forComprehensive Community Support Services (CSSS).Comprehensive Community Support Services(CCSS) certificationthrough the State are required within 90 days of hire. Krossroadswill provide travel expenses to ABQ for the training, if needed.CCSS/case management is the backbone of our treatment model for theKrossroads Integrative Health and Recovery Solutions behavioralhealth and substance abuse clients. In this position you will beworking with a multidisciplinary team of licensed professionals toprovide assistance and support to help rebuild social foundationswith clients. Certifed Peer Support Workers (CPSW) and person inrecovery (minimum 3 years) are encouraged to apply.DutiesKnowledge, Skillsand Abilities:* Implements agencies program for “FullCircle Continuity of Care and Parallel Service Models”, policiesand procedures* Must be able to follow the“Comprehensive Community Recovery and Re-integration (CR2)” programfor legal referrals* Implement the CR2 Justice InvolvedServices (JIS) for all legal referrals* Coordinate withthe JIS Program Manager for all CR2 services provided to clientsand successfully submit compliance reporting to legal programmanager in a timely manner* Provides Intake, Screeningand coordinates with medical staff for assessments*Manage caseload appointments to reduce or eliminate,"no-shows"* The CSW interacts professionally withagency staff, community partners, and community resources asnecessary* Build a collaborative relationship withupper management* Provide 30 hours of billable servicesa week* Provide all case notes within 24 hours ofappointment* Collaborate with administrative team toschedule clients for all services* Provide directservices to clients, including but not limited to: skilldevelopment, building on strengths, identifying natural supports,modeling positive social skills, increasing health and well-being,and any additional support as needed for CSWs and client caseload* Ensures all CCSS services provided are 60%community-based at minimum* Assist client to establishpositive relationships in the community* Provideregular home visits to clients on caseload; assist team with homevisits as necessary* Ensures client and staffsafety* Supports client management of appointments andcommunity engagement* Follows agency incident reportingprotocol* Coordinate wrap around services in thecommunity* Refer clients to resources*Coach clients to learn daily living skills thereby supporting aleast restrictive care setting* Provide, education, andsupport to maintain medication and other treatment regimens to bothclients and staff (e.g. a CSW does not handle medicationdirectly).* May lead educational/skills groups forclients in areas such as basic living skills* UtilizesVocational Rehabilitation to support client’s employmentgoals* Empowers each client to make his/her own lifedecisions* Complete needs assessment, and updates asrequired* Complete and updates CCSS treatment plan asneeded, or within no less than a 90-day timeframe*Assists clients to formulate and achieve goals*Participates in treatment team and clinical meetings*Provides family education and support* Completeadditional documentation as needed including but not limited tocrisis plan, advanced directive, and relapse prevention plan,compliance plan for CR2 program clients* Maintainshealthy boundaries with client, and others that may be involved intheir lives* Is physically able to complete jobduties* Proficiency at digital literacy includingMicrosoft Word, e-mail, telemedicine, and EHR (electronic healthrecord) and guide team in these areas as needed* UsesSeeking Safety curriculum as a tool to support client empowerment,safety, and insight to increase progress towards goals asapplicableQualifications* Mustbe 18 years of age or older.* Have a high schooldiploma or equivalent* Previous history working inbehavioral health and substance abuse* Excellentcommunication skills verbally and writtenJob Type: Full-timeBenefits:* Health insurance* Paid time off*Professional development assistance* VisioninsuranceSchedule:* 8 hour shiftWork Location: Hybrid remote in Albuquerque, NM87102
