Data Entry Records Specialist

Position Summary:

Responsible for analyzing medical documentation, then extracting and data entering specified data elements from external providers and hospital ER and Inpatient records into the electronic health record.

Compensation is dependent on skills and experience.

Your contribution to the team includes:

  • Complete accurate data entry of specified data elements from consults, diagnostic reports, hospital records, and other documents identified and ordered by HealthPoint providers in a timely manner and in accordance with established productivity standards.
  • Retrieve retinal exam results from the EYEPacs website and incorporate them in to the EMR per established procedure.
  • Retrieve mammogram results from Assured Imaging website and downloads them for filing by the fax server team.
  • Participate in the team's daily huddle and collect necessary data for managing staff resources, productivity and volume tracking.
  • Possess and understand basic LEAN knowledge and be able to recognize a process that could benefit from review using LEAN tools including time observations, takt time calculation worksheet, and percent load chart.
  • Participate in New Provider Orientation giving providers necessary information about document preparation and all data entry processes on NextGen training Day 1; Accurately sort, prepare and scan outside records received in the department into a pre-identified standard record set and completes data entry in the appropriate templates for both current established and new patients.
  • Assist the Health Information Scanning Team with document filing as time permits.
  • Participate in meetings and workgroups that could include the health center staff and Health Information Manager regarding data entry processes.
  • Maintain the confidentiality, security, and physical safety of the patients' health and financial information.
  • Consistently follows HIM policies and procedures and contributes to the overall efficiency of the Health Information Department. Makes recommendations to improve HIM services.
  • Maintain good attendance, is punctual and works full scheduled shift as a condition of employment.

  • Attend staff meetings, in-service meetings and participate in agency committees and task force activities as required.
  • Participate in regularly scheduled Medical Records Team Meetings.
  • Performs other duties as assigned by supervisor.

Must have's you'll need to be successful:

  • Associates degree (AA) from two-year college or (BA, BS or BSN) from four-year college or university in medical field and Medical Terminology required or equivalent combination of education and experience.
  • Must have the ability to read and understand incoming EHR correspondence as well as the ability to read patient medical charts.
  • Ability to recognize and assign accurate document types is required.
  • Must have the ability to read and interpret technical and other complex documents.
  • Must have the ability to write routine correspondence such as letters and memos.
  • Must have the ability to present information in one-on-one and small group situations to internal and/or external clients.
  • Work situations require consideration and interpretation of circumstances or information to choose the most effective response or solution. Solutions may be technical, yet relatively straightforward and well-defined once problems are understood.

Proof of vaccination for COVID-19 is required, prior to start. HealthPoint does not accept the Johnson & Johnson COVID-19 vaccine as proof of vaccination. If you have received the Johnson & Johnson vaccine, we ask that you provide documentation demonstrating proof of an alternate COVID vaccine or vaccine series. All new employees are also required to show proof of immunizations and/or immunity to MMR (measles, mumps, rubella), Varicella, annual Influenza and TB QuantiFERON Gold Titer. Additionally, if you work in a HealthPoint clinic, Tdap (within last 10 years) is required. Hepatitis B. is required for clinical employees with potential exposure to blood/blood products. All immunizations are a condition of employment. Upon hire, employees must provide proof of their immunizations and/or immune titer results prior to starting or no later than their fifth (5) business day of employment.

Where to gather your records:

  • If you are providing immunizations from an electronic health record, please ensure that you obtain a copy of your full records rather than a screenshot. Each page of your records should include your first and last name, date of birth, and the name of the health system from which the records are pulled.
  • If records do not show any data, please seek guidance from your provider for further assistance.
  • If you are unable to provide proof as noted above, you can choose to have a lab titer drawn to check immunity or to be re-vaccinated. If you receive vaccination(s) or lab titers, you may obtain them through HealthPoint at no cost to you. This is a great opportunity to get your immunization record up to date at no additional expense.
