Job DescriptionBe a part of a world-class academic healthcare system UChicago Medicine Ingalls Memorial Hospital as an Department of Human Services (DHS)/ Fitness Restoration Program Coordinatorin theBehavioral Health department. This position is a 100% onsite opportunity. You will need to be based in the greater Chicagoland area.The DHS Fitness Restoration Program Coordinator provides general oversight of the specialty Fitness Restoration Program in collaboration with the Nursing Patient Care Manager. The Fitness Restoration Coordinator ensures compliance with quality and regulatory standards communication with the Department of Human Services (DHS) and management of the legal processes.Essential Job FunctionsCollaborates with Behavioral Health Leadership to ensure all regulatory compliance clinical competency and educationalrequirements are metCreates a learning environment that encourages staff to maximize level of professional practiceDesigns and continuallyevaluates core competencies int he implementationof a Therapeutic community model of careRegularly meets withthe patients community leaders to promote a therapeutic healing and empowering cultureActs as the primary facilitatorfor daily community meetingsEnsures the behavioral incentive program for DHS patients is functional and consistently maintainedEnsures patients have a minimum of one hour time outdoors (weather Permitting)Observes Clinical Therapist RN an BHT lead groups and provides feedback and education to build skills as appropriateAudits EMR for quality compliance with group programming and completeness forclinical documentationDesigns and implements a professional development program for core DHS staffPromotes a culture of Employee EngagementFacilitate appropriate admissions tot he program with the goal of stabilizing the unit censusCoordination with the accepting attending Psychiatrist and nursing staff regarding the needsof the patientCommunication with the referring county authorities to plan transportation to DHS program which includes establishing the transport date and accurately composing the transport authorization lettersReview referral packets from DHS for individuals determined to be Unfit to Stand Trial (UST)Participate in the weekly meetings with DHS leadershipRequired QualificationsMasters Degree in Clinical Behavioral HealthMinimum of 3 years experience in a Behavioral Health Care environment preferred inpatient BH Forensic Clinical experienceLicensed ClinicalSocial Worker (LCSW)Current BLS with American Heart AssociationCurrent CPIPreferred QualificationsLicensed ClinicalProfessional Counselor (LCPC)Position DetailsJob Type/FTE: Full Time 8-hour shiftsShift: DaysWork Location: OnsiteUnit/Department: Behavioral HealthCBA Code: Non - UnionWhy Join UsFor nearly a century Ingalls Memorial has pioneered sophisticated clinical care and developed the areas most convenient network of comprehensive outpatient centers all dedicated to improving the health and wellbeing of the community.
