This position is designed as Hybrid (In office and Remote). This position requires a criminal background check. Therefore, you may be required to provide information about your criminal history in order to be considered for this position. WORKING TITLE: Transition Facilitator Qualifications Education and Experience: Graduation from an accredited college or university with a bachelor's degree in special education, education, rehabilitation or therapeutic health, nursing, sociology, social work, or any behavioral science field and experience equivalent to two years of working with persons with disabilities, aging, and/or mental health work.Substitution of Education for Experience: Additional graduate coursework in special education, rehabilitation or therapeutic health, nursing, sociology, social work, or any behavioral science field may be substituted for the required experience on a year-for-year basis to a maximum of four years.Substitution of Experience for Education: Qualifying professional intellectual and/or developmental disabilities, aging, or mental health work may substitute for the required education on a year-for-year basis to a maximum of four years.Education and Experience: Graduation from an accredited college or university with a bachelor's degree and experience equivalent to three years of working with persons with disabilities, aging, and/or mental health work.Education and Experience: Graduation from an accredited college or university with a bachelor's degree and completion of an internship with the Department of Disability and Aging.Necessary Special Qualifications: Complete a background check in a manner approved by the appointing authority.Some positions may require a valid driver's license.Overview Under general supervision, is responsible for professional intellectual and developmental disability (IDD) and aging services monitoring work of considerable difficulty; and performs related work as required. An employee in this class monitors plans of care for people with intellectual and developmental disabilities and/or the aging population that reside in community settings to ensure compliance with federal and state requirements. Work involves providing technical assistance, performing on-site visits, recording and reporting findings, analyzing information, and making recommendations for improvement.Responsibilities Monitors environments of persons supported, contracted provider agencies, and state-operated community homes to ensure compliance with policies, procedures, litigation requirements, and state/federal rules and regulations to ensure health and safety.Provides technical assistance to external stakeholders as mandated or requested for compliance with federal, state, and departmental rules, regulations, policies, procedures, and litigation requirements.Provides information to other departmental staff for compliance with federal, state, and departmental rules, regulations, policies, procedures, and litigation requirements.Meets with staff and contracted provider agencies to gather information for person-centered planning for person-directed planning, to provide technical assistance, and to follow up on complaints and verify accuracy of received information.Documents visits, findings, training details, technical assistance questions and answers, activities, contact logs, and leave and attendance.Schedules trainings, technical assistance, meetings, and provider-related reviews to educate, inform, and ensure compliance.Attends off-site events as needed.Competencies (KSA's) Competencies: Decision QualityCustomer Focus Optimizes Work ProcessesCommunicates EffectivelySituational AdaptabilityKnowledge:ClericalCustomer and Personal ServiceEnglish LanguageTherapy and CounselingPublic Safety and SecuritySkills:Time Management WritingCritical Thinking MonitoringSpeakingAbilities:Written ComprehensionInductive Reasoning Problem Sensitivity Auditory Attention Speech Clarity Tools & Equipment Mobile PhoneVehicleComputer/LaptopPrinter/Copier
