Schedule: Fulltime Monday through Friday shifts start at 8am weekends offCompensation: Pay range from $62000-$72000 annuallydepending on dialysis experienceBenefits:Support and mentorship opportunities available through peer and corporate training for individuals new to renal careComprehensive medical dental and vision benefitsLife and long-term disability insurance provided at no additional expense to employeePaid time off (PTO) including holidaysExtended Sick Bank (ESB) in addition to PTO paid time for doctor appointments sickness or medical leaveRetirement plans with $.50 of each contributed dollar matched for eligible employees up to 8 percentEmployee assistance programWellness programAmong othersResponsibilitiesWhat You Can Expect:Provide patient support to address emotional financial and lifestyle changes associated with chronic kidney diseaseAssist patients with utilizing community resources for nutritional emotional transportation and housing supportAdvocate and assist patients with accessing healthcare and other benefitsEducate patients and their families regarding end-stage renal disease treatment options including dialysis transplant and medical managementHelp patients compare treatment options empowering them to make informed decisions about their careCoordinate translation services for patients and caregiversAssess family and support system and refer for additional services as neededProvide counseling services to patients and their families directed at helping them cope with kidney failure and dialysis follow the treatment plan and achieve the patients rehabilitation goalsDevelop and implement individualized care plans with interdisciplinary care team based on psychosocial assessment to help patients enjoy a better quality of lifeCollaborate with interdisciplinary care team to assess ongoing needs and modify care plan as necessaryEducate interdisciplinary team members about emotional aspects of dialysisAdvocate for patients at clinic community and state/federal levelsServe as ESRD Network liaisonEngage in a data-driven quality improvement process to positively impact outcomesQualificationsSuccessful Candidates Bring:Excellent communication skillsActive listening skills to understand patient and family concernsDesire to collaborate with care teamsAbility to problem solveEducation/Training:Masters Degree in Social Work (MSW) requiredLicensed in the state of practice as required by state regulationsNew MSW graduates welcome job shadow opportunities availableRenal care experience preferred not requiredDCIs Differentiator:Since opening the first clinic 50 years ago in Nashville Tenn. our Dialysis Clinic Inc. family has grown to be the nations largest nonprofit dialysis provider with more than 270 locations in 30 states serving nearly 14000 patients each day.
