Discharge Care Navigator (Full Time, Baltimore Washington Campus) at Sheppard Pratt Careers

General Summary:In collaboration with the patients interdisciplinary treatment team performs a full range of aftercare and discharge planning services geared to mentally ill patient population.Maintains comprehensive knowledge base of agencies organizations and resources within the referral community for discharge planning and placement.Communicates with patients based on an understanding of the developmental needs of the population.Develops and assists in the implementation of discharge and aftercare plans; works with patients and families to facilitate the community transition and assure appropriate match to patients needs.Knowledge Skills and Abilities Required:Bachelors degree in a related mental health field such as Social Work Nursing Psychology or a Rehabilitation Services specialty.For internal candidates high school education plus an additional 5 years of relevant experience (2 years of work must be within SPHS) may be considered a substitute for a bachelors degree.Six to twelve months of relevant field or work experience in a psychiatric setting.Benefits:At Sheppard Pratt you will work alongside a multi-disciplined team led by a bold vision to change lives.
