Discharge Planner, Per Diem at University of Miami

This role works collaboratively with the Social Worker and the Case Manager covering the clinical area.CORE JOB FUNCTIONS1. Processes the physician order based on the patients choice and based on insurance contract agreements with certified agencies.2. Coordinates all Home Health Durable Medical Equipment Orthotics and Infusion.3. Utilizes electronic referral system to send referrals for all post discharge needs to licensed providers.4. Assures follow-up with companies for the coordination of benefits and services.5. Obtains any required forms for completion with the physician attending qualified practitioner or ordering physician for completion.6. Assists the social worker with obtaining patient choice when warranted.7. Reviews Medical Record electronically to obtain appropriate documentation for post discharge needs and coordinate services.8. Communicates the finalization delivery and acceptance of case to the nursing and care coordination staff.9.
