Evening Shift Licensed Practical Nurse (LPN)

As an Evening Shift Licensed Practical Nurse (LPN) on our team, you will play a vital role in delivering exceptional patient care while making a positive impact on our residents' lives.

Job Summary:

We are seeking a dedicated and compassionate LPN to join our evening shift team. As an LPN with Bradford Manor Nursing and Rehabilitation, you will provide high-quality nursing care to our residents, participate in assessments, develop and implement individualized care plans, and collaborate with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:

  • Provide direct patient care to residents, including administering medications, wound care, and assisting with ADLs

  • Conduct initial and ongoing assessments to identify resident needs and develop individualized care plans

  • Implement care plans, monitor progress, and communicate with residents, families, and healthcare team members

  • Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
    *Maintain accurate and timely records of resident care and progress
    *Participate in Quality Assurance and Performance Improvement initiatives
    *Contribute to a positive and supportive work environment

Requirements:

  • Current LPN licensure in the state of Pennsylvania

  • High school diploma or equivalent required; LPN diploma or degree preferred

  • Previous LPN experience in a long-term care setting preferred

  • Ability to work effectively in a fast-paced environment with minimal supervision

  • Excellent communication and interpersonal skills

  • Strong attention to detail and organizational skills

  • Ability to maintain confidentiality and adhere to HIPAA guidelines
