Growing togetherAs a Field-Based Case Manager (CM), you will act in a liaison role to ensure appropriate care is accessed, as well as to provide home and social assessments and member education. Case Managers work in a team based structure and spend the majority of their time in the community engaging directly with members.If you reside within a commutable distance of Pickaway, Fairfield, Ross, Fayette or Madison Counties, OH, you will have the flexibility to work remotely* as you take on some tough challenges.Primary Responsibilities:Engage members either face to face or over the phone to assist with closing gaps in care, linking to necessary services and providing education about their healthReview available member service records and relevant documentation (e.g. utilization history, functional level, stratification information, current plan of care)Conduct member health assessment that includes bio-psychosocial, functional, and behavioral health needsUtilize interviewing techniques and active listening to collect and retain member information and incorporating responses as they are presented to complete assessmentIdentify member service needs related to health concernsIdentify urgent member situationsEngage member to participation in the assessment process and collaboratively develop Care Plan based on individual needs, preferences, and objectivesProvide nursing oversight while collaborating with Care Guides (CGs)Work with members to develop healthcare goals and identify potential barriers to achieving healthcare goalsIdentify member support systems available and incorporate into their Care PlanReview plan benefits and identify appropriate programs and services based on health needs and benefitsIntegrate health care and services needs into a plan or recommendation for member care and serviceWork collaboratively with the interdisciplinary care team to ensure an integrated tam approachCollaborate with member to create solutions to overcome barriers to achieving healthcare goalsIdentify relevant community resources available based on member needsRefer members to appropriate programs and servicesFacilitate member choice of preferred providerAdvocate for individuals and communities within the health and social service systems You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:State of Ohio RN License, and applicable degreeField-based experienceKnowledge of culture and values of communityFamiliarity with the resources available in the communityResided within the local community for 2+ yearsReside or be able to commute within Jefferson County, OhioAbility to travel locally up to 75% of the timeAccess to reliable transportation that will enable you to travel to member and/or patient sites within a designated areaAvailable to work Monday through Friday 8:00 AM to 5:00 PMHave a dedicated work area established that is separated from other living areas and provides information privacyProven ability to keep all company sensitive documents secureLive in a location that can receive UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet servicePreferred Qualifications:Case Management Certification (CCM)Care management experienceExperience working in Managed CareTelephonic customer service experienceKnowledge of Medicaid and/or Medicare populationKnowledge and/or experience with behavioral health or substance use disordersEmployment in a medical office or other provider environment *All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.27 to $50.48 per hour based on full-time employment.
