Growing together.As a Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social screenings, care coordination and member education.Travel 50% in Buncombe County, NC , NC and surrounding areas within 120 miles.Schedule Monday-Friday standard business hours. No nights, no weekends and no holidays. We offer our established staff the option to work 4 10-hour days (7 am-6 pm) in lieu of the traditional 8 hour 5/day week schedule. *after employee has demonstrated competency with the role and are able to meet metrics, etc.This position is filed-based with a home-based office. Primary Responsibilities:Linking members to applicable community resourcesComplete field visits for screenings and preventative health educations when applicableTelephonic outreach to targeted membersKeep the member out of the hospital by supporting regular visits to their primary physicianKeep member actively engaged with their primary physicianSupport the member to ensure pick - up of their RxProactively engage the member to manage their careProvide member educationSupport transitions of careCreate a positive and engaging experience for the memberHelp to keep members compliant with their care plansCollaborate with member's care team (community, providers, internal staff)Partner with and support care team by completing delegated tasksKnowledge and continued learning of community cultures and valuesConduct individual-level screenings to capture member's current health care needs (e.g. Transitions of Care, Health Risk assessments)Engage member to participate in the screening process and collaboratively develop plan of care based on their individual needs, preferences, and objectivesParticipation in community events relating to preventative healthYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications:High School Diploma/GED (or higher)1+ years of experience with familiarity with the resources available in the communityMust have a designated workspace inside the home with the ability to access high-speed internetAccess to reliable transportation that will enable you to travel to client and/or patient sites within a designated areaAbility to travel locally (up to 50%), up to 150 miles round trip (75 miles one way) (includes mileage reimbursement)Must be a NC resident and have resided within the local community for 2+ yearsPreferred Qualifications:Bachelor's Degree (or higher) in Social Work and / or Health Care AdministrationLicensed Practical Nurse (LPN), Certified Nursing Assistant / Home Health Aide, Medical AssistantCHW AccreditationExperience working in Managed CareKnowledge of Medicaid population*All Telecommuters will be required to adhere to UnitedHealth Group's Telecommuter Policy.
