DescriptionWe are searching for a Field Relations Coordinator (Star Kids) -- someone who works well in a fast-paced this position you will be responsible for deployment of team-based population management including developing and maintaining positive relationships with physicians members and their families and providers. Position interacts with physicians members caregivers and care coordination team on a daily basis. Position is responsible for assessments coordination activities referral and linkage to community resources monitoring and tracking outcomes.Service areas will be in the Galveston/Houston/Tyler area(s)Position will be hybrid between office/homeThink youve got what it takesKnowledge of managed healthcare environmentExceptional written and oral communication skillsMinimum RequirementsHigh school diploma or GED requiredDrivers license and current auto insurance required (very little driving involved)5 years experience in health care local and state programs for children with disabilities or managed health care setting requiredBachelors degree may substitute for four (4) years of required work experienceMasters degree may substitute for the five (5) years of required work experience**Authorization to Release Personal Information must be signed prior to beginning work in this role. As a remote worker/road warrior with Texas Childrens Health Plan this consent allows Texas Childrens to disclose an employees home address for the purposes of geomapping**Preferred QualificationsCHW (Community Health Worker certificationCase management experienceBachelors DegreeBilingual in SpanishResponsibilitiesDevelops and maintains satisfactory business relationships with participating providersAssists participating providers with referrals to community resources needed engagement with schools and/or communication between providersReviews and interprets all relevant information with member/family and develops/initiates a care plan and self-management plan with family during an in-home assessmentIdentify options and initiates collaboration with referral sources to ensure Member/family linkage to specialists homecare providers financial assistance housing meals transportation and other community resources as appropriate for Members with unmet needsIdentify options and initiate collaboration with referral source to ensure Member/family linkages financial assistance legal aid housing job placement and other community resources as appropriateProvides for resolution and revision of plans on an ongoing basis as needs are identified additional information is required or problems are encounteredEnsures thorough communication and education to members/providers an understanding of delivery system capabilities to improve access and address member needsAs part of our commitment to maintaining a safe and healthy workplace all successful candidates will be required to undergo respiratory fit testing in compliance with occupational health and safety standards.
