Job TitleCommunity Support SpecialistOur Community Support Specialist plays a vital role in promoting rehabilitation, recovery, and resiliency for individuals and families. We are seeking a skilled professional to join our team as a Community Support Specialist.Job DescriptionWe are looking for a highly motivated and compassionate individual who can work effectively with individuals and families to identify strengths, address barriers, and develop skills necessary for independent functioning in the community. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work collaboratively with medical providers, multidisciplinary team members, and community resource providers.The successful candidate will be responsible for:Coordinating and providing services and resources to individuals/families to promote rehabilitation, recovery, and resiliency;Identifying and addressing strengths, needs, and barriers to attaining self-identified goals;Conducting ongoing assessments to determine if services accessed are meeting or have adequately met the individual's needs;Assisting individuals/families in accessing community resources, financial assistance programs, and services that are needed;Facilitating development of skills in activities of daily living, interpersonal coping, socialization, and community functioning;Enhancing professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings, and workshops;Documenting all client encounters and submitting billing documentation as appropriate;Maintaining current and comprehensive client files; andReporting potential or actual patient safety concerns, medical errors, and/or near misses in a timely manner.Required Skills and QualificationsEssential qualifications include:Bachelor's Degree in a related field;1 year directly related experience;CPR for Healthcare/BLS Provider certification within 30 days;Valid New Mexico Driver's License;UNM Vehicle Operator's Permit within 60 days;Ability to lift up to 20 pounds occasionally and/or up to 10 pounds frequently and/or a negligible amount constantly;Physical demand requirements are in excess of those for Sedentary Work.BenefitsOur organization offers a competitive compensation package, including a salary range of $22.49-$33.74 per hour, depending on experience.
