Job Details LSC Raleigh Office - Raleigh NC Fully Remote Full Time $48000.00 - $50000.00 Salary None First ShiftDescription The Foster Care Case Manager has primary responsibility for behavioral interventionmanagement and coordination of services for the youth. He/She works intensely with the clientand foster parent/care giver by providing ongoing support. If applicable within program alsoserves as a supervisor and consultant to licensed therapeutic foster parents (contractors) whocarry out the therapeutic interventions for the youth in care.Must reside in Raleigh NC and surrounding area.The Case Manager serves as a member of a regional foster care clinical team. Under theguidance of a Foster Care Supervisor if applicable the case Manager will recruit and licensetherapeutic foster parents; all team members manage regional team/agency specific operationswhich include facilitating placement goals team goals budgeting marketing communityrelations and promoting Lutheran Services Carolinas within the child welfare community.Essential Functions1. Plan and supervise the placements for youth and foster families. Facilitate the setting ofplacement goals and desired outcomes established by the placement team.2. Write and monitor behavioral intervention treatment plans monitoring progress accordingto the level of care and documentation and planning transitions for discharges.3. Possess strong knowledge of the Collaborative Problem Solving to ensure the model isimplemented in the home if applicable to the program.4. Maintain documentation in the youths file and foster parent file according to state andfederal standards and program guidelines.5. Facilitate the recruitment assessment training and licensing of therapeutic and non-therapeutic foster care homes according to the established agency policy accreditationstandards and state licensing rules.6. Responsible for assessing for symptoms or implementing interventions to help fosterparents address challenging behaviors in youth and adolescents.7. Partner in facilitating placement matches. Consult with team members assess appropriateresources for youth and foster families and assist with matching. Assist with obtaining allplacement paperwork which includes clinical information and contractual documentation(financial agreements and medical necessity documentation if applicable).8. Complete all placement and attendance forms as required to facilitate billing.9. Complete foster care reviews with governing agencies. Complete annual evaluations onlicensed foster parents. Deliver ongoing support and supervision to licensed therapeuticfoster parents who carry out therapeutic interventions for the youth in care.10. If applicable within program facilitate a monthly support group for foster parents. Provideconsultation and collaboration with foster parents and therapist on identifying problemareas developing plans for addressing those problems in order to minimize the risk ofplacement disruption. Provide ongoing training in order to continue to develop the skilllevel of foster parents. Provide on call support to foster families on a regular basis.11. Maintain up-to-date documentation as related to foster home licensure contractualagreements and all agency and regulatory guidelines. Documentation must be completedand maintained at all times and in accordance with contractual standards and timeframes.12. Assist in marketing Lutheran Services Carolinas programs with potential referral sources.Must have the knowledge and ability to respond independently and promptly to marketingand community relations needs.13. Liaison on behalf of the consumer by ensuring all applicable services are connected andauthorized; including assistance with immigration procedures coordinating legal activity forclients and identifying attorney representation as needed. Develop and maintainconsumers PCP and facilitate child and family team meetings.14. Serve on committees and projects at Lutheran Services Carolinas if applicable.15. If applicable be knowledgeable of DHS policy and procedure in terms of intakeprocedures service and reporting procedures. Maintain accurate up-to-date information onfoster care children and comply with LSC/DHS documentation and reporting requirements.16. Respond to other duties as requested.Qualifications Education: Bachelors degree in a human services related field and two years of experience withpopulation served or; Bachelors degree in a non-related field and 5 years of experience withpopulation served or; Masters degree in a human services related field and one year ofexperience in child welfare.Specific skills/abilities: Proven ability to work in a self-directed manner that will ensure that allwork obligations are met according to the requirements of this position. Strong skills in groupand team facilitation completing assessments and writing behavioral treatment plans. Timemanagement and organizational skills required. Must be flexible and be able to work irregularhours. Knowledge of Microsoft office products Word Excel etc. Demonstrated verbal andwritten communication skills.Specialized knowledge licenses etc: Must be at least 21 years of age possess a valid driverslicense and have reliable transportation. Must have knowledge of family systems childhooddevelopmental and attachment theory psychopathology behavior management psychotropicmedications. If applicable prior work experience with refugee immigrant or minority families anddemonstrates cross cultural sensitivity. Knowledge of state community and agency resourcesfor all foster care programs..Working Conditions/Physical Requirements: (Reasonable accommodations may be made toenable individuals with disabilities to perform the essential functions).1. Position may be home based (TBD by location).2. Ambulatory in all locations ability to navigate steps.3. Ability to bend stoop and reach overhead.4. Minimum lift and carry requirement of 25 pounds.5.
