Are you seeking a Case Management position Come work for Springwell and help older adults age-in-place and adults with disabilities receive care in the community.The compensation for this role is $28.21/hr for a 37.5 hour work-week.DESCRIPTION:Work collaboratively as part of an interdisciplinary team that provides in-home services to older adults and individuals with disabilities.
