Nursing Home Rounding Physician Los Angeles, CA
We are seeking a dedicated and compassionateNursing Home Physician to provide ongoing medical care for residents across oneor more skilled nursing facilities in the Los Angeles area. The physician, also known as a SNF-tist, willconduct regular rounds, manage acute & chronic conditions, collaborate withinterdisciplinary teams while ensuring highquality, patientcentered care for an aging population with complex medical needs.
ROLE OVERVIEW
Begin @Skilled Nursing Facilities; can add Long Term Acute Care when ready
Primarilyweekends will be off. Physicians make own schedule
Your careteams Mid levels also cover each of the facilities. Physicians supervise Mid-levels. Physicians see admits within 72 hrs to stay in compliance
Covermultiple facilities each week under the model of Round and Go
Covermultiple facilities each week under the model of Round and Go. Physicianschoose 1 facility to visit every day. IF there is an emergent need for apatient to be seen or a difficult family then will be added to calendar ASAP
CALL: Midlevels cover all phone calls/ text messages. MD/DOs only called when anabsolute emergency. Low patient panel
Communicateand provide timely reports to private practice leadership as needed
DUTIES
Conduct routine + asneeded rounds on nursing home residents, ensuring timelyevaluation & followup.
Manage acute medical issues, chronic disease progression &posthospitalizationcare.
Perform comprehensive assessments, including history, physicalexamination, medication review, and care planning.
Coordinate care with nursing staff, therapists, socialworkers, case managers & facility leadership.
Communicate effectively with families + caregivers regardingpatient status, goals of care & treatment options.
Review + sign off on orders, labs, imaging where available &therapy recommendations.
Ensure compliance with state, federal & facilityregulations, including documentation standards.
Participate in quality improvement initiatives, readmissionreduction efforts & interdisciplinary meetings where available
Provide guidance on advance care planning, palliative careneeds, and transitions of care

YOU
MD or DO degree from an accredited institution.
Boardcertifiedor boardeligiblein Internal Medicine, Family Medicine, or Geriatrics
Active California medical license (or ability to obtain)
Experience in skilled nursing facilities, longterm care, geriatrics, or postacute care preferred
Strong communication, collaboration & clinical decisionmaking skills
Ability to work independently across multiple facilities
Board Certified or Board Eligible Medical Doctor with or w/o US residency with current US work visa.
