Home Care Certified Nursing Assistant (PRN) at Hospice and Palliative Care of the Piedmont

Works cooperatively with other team members to effectively implement the Hospice Plan of Care for patients.REPORTS TO: Home Care Clinical Director QUALIFICATIONS:Must be empathetic and responsible able to use time effectively be able to accept supervision relate well to patients and understand and accept the hospice philosophy.Current South Carolina Nurse Aide Certification.Demonstrate competency in skills needed to perform job duties. Ability to function as part of a teamAbility to embrace the mission and culture of the organization.RESPONSIBILITIES:Assist patients with baths care of mouth skin and hair.Assist patients with elimination functions.Help patients in and out of bed; assisting with ambulation.Perform such incidental household services as are essential to patient health care. Report any changes or concerns regarding patients conditions and/or family situations to the registered nurse.Maintain effective working relations with patients families and members of hospice interdisciplinary team. Perform other related duties as assigned. Document all aspects of care provided according to the policies established for the Hospice/HomeCare program and the individual patients care plan.
