Our team is committed to partnering with our seniors to remain at home through promoting open communication and shared decision making while providing excellent care and services.ResponsibilitiesDay-to-day responsibilities include:Visiting members in their homes and supervising approved care/servicesCollecting important information and updatesCommunicating member status and other important information to doctors nurses and other caregivers at Senior LIFEConducting ongoing periodic assessments ensuring the current care plan is still the best course of actionAttending family and Senior LIFE team meetings to update participants on members status and needsTraining member assistant aidesQualificationsThis role requires significant in-home work with an elderly/frail population so candidates should be comfortable with this kind of hands-on work. Applicants should have at least two years experience in a similar role/field with a valid Pennsylvania drivers license.
