This position is responsible for managing all new patient referrals, coordinating with healthcare providers, and ensuring a seamless intake process.Key Responsibilities:Receive and process referrals from hospitals, physician offices, and other referral sourcesVerify patient insurance coverage and eligibilityObtain prior authorizations as requiredCoordinate start-of-care visits with clinical staffMaintain accurate documentation in electronic medical records (EMR)Communicate with patients, families, physicians, and insurance providers as neededEnsure compliance with HIPAA and other regulatory requirementsQualifications:High school diploma or equivalent; Associates or Bachelors degree preferredExperience in a home health or healthcare office setting strongly preferredKnowledge of Medicare, Medicaid, and private insurance guidelinesFamiliarity with EMR systems (e.g., WellSky, Kinnser, Homecare Homebase)Strong communication and organizational skillsAbility to handle confidential information with discretionAt least 5 years experience in Home Health Intake/SchedulerBenefits:Competitive payHealth, dental, and vision insurancePaid time off and holidaysOpportunities for professional growth
