Position: PT - Home HealthPosition SummaryProvides physical therapy services in clients homes as prescribed by the physician and offers consultation and education to agency staff.Level of ResponsibilityWorks under agency policies and client care plans. Reports to the Argus of Colorado administrator or designee and supervises assigned personnel.ResponsibilitiesPatient Care:Conduct initial and ongoing evaluations including OASIS assessments.Develop and update care plans with the physician and care team.Identify barriers set goals provide treatment education and equipment.Coordinate services and plan discharge.Evaluate care effectiveness.Documentation:Record evaluations treatments and client responses promptly.Document conference outcomes and quality assurance data.Attend care conferences staff meetings and in-service programs.Provide education and consultation to clients families and staff.Supervise aides therapy assistants and paraprofessionals.Maintain professional knowledge through education professional organizations and workshops.Perform other related duties as needed.Qualifications : Active Colorado PT licenseGraduate of a physical therapy curriculum approved by the American Physical Therapy Association (APTA) or any accrediting body approved by the U.S. Office of EducationExperience in Home Health preferred but not requiredValid Drivers License & clean Motor Vehicle Record (MVR)Additional Information : Benefits:PAID training!PRNFlexible scheduleWork - Life balance no overnight services401kSalaryPRN benefits plusMedical Dental Vision SupplementalPaid Vacation & HolidaysCompany vehicle may be offered but not assuredPay: PRN $75 - $85 per visitSalary $75000 - $90000Remote Work : NoEmployment Type : Full-time Key Skills Environmental Safety,Feed,Lotus Notes,Administrative Services,Activex Experience: years Vacancy: 1
