Home Health Skilled Nurse (LPN) – Full Time

Hanford Home Health | Tri Cities, WA

Employment Type: Full Time (36–40 hours per week)

Job Overview:

Hanford Home Health is seeking a compassionate and skilled Full Time Home Health Skilled Nurse (LPN) to provide in-home, skilled nursing care to EEOICPA (Department of Labor) benefits recipients and other medically complex patients. As a key member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Schedule & Shift Structure:

  • Full time status is 36–40 hours per week

  • Shifts range from 1 to 12 hours, based on patient needs and census

  • Full time staff are primarily scheduled for 12-hour shifts

  • Alternative shift lengths may occur only by mutual agreement

  • Day Shift: 7:00 AM – 7:00 PM

  • Night Shift: 7:00 PM – 7:00 AM

Key Responsibilities:

  • Provide skilled nursing care in accordance with the authorizing practitioner’s orders and the individualized Plan of Care

  • Perform nursing assessments and ongoing monitoring within LPN scope of practice

  • Administer medications and treatments; monitor for side effects and changes in patient condition

  • Provide skilled interventions as ordered (including wound care and other therapeutic interventions when within scope and ordered)

  • Measure and document vital signs, intake/output as applicable, pain, weight, and patient responses per the Plan of Care

Required Qualifications:

  • Active Washington State LPN license in good standing

  • Graduate of an accredited School of Nursing

  • Minimum of one (1) year of nursing experience; home health experience preferred

  • Current CPR/BLS certification for healthcare professionals

  • Washington State Food Worker’s Card (or ability to obtain upon hire)

  • Valid driver’s license, reliable transportation, and auto insurance

  • Ability to pass background check and drug screening

Preferred Qualifications:

  • Experience in home health, hospice, or geriatric care

  • Prior experience with electronic documentation systems

Pay & Benefits:

  • Pay Rate: Licensed Practical Nurse (LPN): $45.00 per hour

  • Base Pay range: $45.00 – $55.00 per hour

  • Actual wage may vary based on assignment, credentials, experience, and applicable shift differentials

  • Full time skilled nurses are eligible for annual wage increases in accordance with agency policies and procedures
