Home Health Skilled Nurse (LPN) – Per Diem

Hanford Home Health | Tri Cities, WA

Employment Type: Per Diem / As Needed
Shift Availability: Day Shift, Night Shift, or Combination
Shift Length: Shifts range from 1 to 12 hours

Job Overview:

Hanford Home Health is hiring Per Diem Home Health Skilled Nurses (LPNs) to provide skilled nursing care to EEOICPA benefits recipients and other medically complex patients. As a Per Diem LPN, you will be part of a dedicated team focused on delivering high-quality, patient-centered care in the comfort of patients' own homes.

Schedule & Shift Structure:

  • Scheduled as needed based on census and coverage requirements

  • No guaranteed hours or fixed schedule

  • Work schedules posted one (1) month or more in advance when applicable

  • Staff are expected to complete all assigned shifts accepted, subject to agency attendance and call-out policies

  • Per diem status does not guarantee hours, ongoing assignments, or priority scheduling

Staffing Model:

  • Care is primarily one to one

  • In limited circumstances, staff may support more than one patient within the same household when authorized by the individualized Plan of Care

Key Responsibilities:

  • Provide skilled nursing care in accordance with the authorizing practitioner's orders and the individualized Plan of Care

  • Perform nursing assessments and ongoing monitoring within LPN scope of practice (LPNs complete focused assessments and report findings to the supervising RN)

  • Administer medications and treatments; monitor for side effects and changes in patient condition

  • Provide skilled interventions as ordered (including wound care and other therapeutic interventions when within scope and ordered)

  • Measure and document vital signs, intake/output as applicable, pain, weight, and patient responses per the Plan of Care

  • Observe and report changes in patient condition to the supervising nurse, RN Case Manager, and/or Director of Clinical Services as required

  • Educate patients and families on conditions, medications, treatments, safety measures, and Plan of Care requirements

  • Collaborate with interdisciplinary team members to coordinate care and support optimal patient outcomes

  • Assist with mobility, personal care, ADLs and IADLs when clinically indicated and outlined in the Plan of Care

  • Complete accurate, timely documentation in the agency-approved electronic health record system

  • Follow infection control, safety, emergency preparedness, confidentiality, and HIPAA requirements

  • Report suspected abuse, neglect, or exploitation of vulnerable adults in accordance with Washington State mandatory reporting laws and agency policy

  • Comply with OSHA standards, including the Bloodborne Pathogens Exposure Control Plan, and utilize required personal protective equipment

  • Complete state-required continuing education requirements per state guidelines

  • Perform only those duties authorized within the applicable LPN scope of practice, licensure, and delegated authority under Washington State law and the agency's policies and procedures

Required Qualifications:

  • The physical and functional requirements listed are considered essential functions of this position

  • Ability to stand, walk, bend, lift, and assist with transfers as required by patient needs and the individualized Plan of Care

  • Ability to lift up to 50 lbs using proper body mechanics

  • Active Washington State LPN license in good standing

  • Graduate of an accredited School of Nursing

  • Minimum of one (1) year of nursing experience; home health experience preferred

  • Current CPR/BLS certification for healthcare professionals

  • Washington State Food Worker's Card (or ability to obtain upon hire)

  • Valid driver's license, reliable transportation, and auto insurance

  • Ability to pass background check and drug screening

  • Strong clinical judgment, documentation, and communication skills

  • Ability to use agency-issued or personal mobile devices for scheduling, communication, and electronic documentation

  • Ability to work safely and professionally in private home environments, which may include pets, exposure to tobacco smoke, limited space, and other residential conditions

Preferred Qualifications:

  • Experience in home health, hospice, or geriatric care

  • Prior experience with electronic documentation systems

Pay & Benefits:

  • Pay Rate: Base rate $45.00 per hour

  • Actual wage may vary based on assignment, credentials, experience, and applicable shift differentials

  • This position is non-exempt (hourly). Overtime is paid in accordance with Washington State and federal law. Overtime pay and applicable shift differentials are paid in addition to the base hourly rate

  • Shift differential pay per agency policy

  • Mandatory Washington meal and rest periods are paid

  • Travel within the Tri Cities and surrounding areas; mileage reimbursed at $0.725 per mile

  • Washington Paid Sick Leave provided in accordance with Washington State law

  • Supportive nursing supervision and leadership team

  • Per diem positions are not benefits eligible except as required by law

  • Washington State Patrol background check and disclosure required at hire and every two (2) years and as needed

  • Driver's Record Request required annually and as needed
