Work 1 on 1 with your patients and establish a rewarding relationship that you wont get in any other healthcare addition Eden Health is a Certified Great Place to Work company 6 years in a row!Job Summary:Provides skilled nursing care ordered by the physician as indicated by the plan of care to all patients on caseload.Review caseload with supervising RN on a regular basis as established by best practice and/or State guidelines.Obtains necessary physician orders and collaborates with the physician as the patients needs dictate.Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.Participates in coordination of hospice services appropriately reporting the identified needs for hospice interdisciplinary group members (e.g. hospice aide RN OT PT MSW ST Dietitian Chaplain or Hospice Manager)Uses equipment and supplies effectively and efficiently.Provides appropriate pain/symptom management.
