The Hospice Social Worker provides education support individual and family counseling linkage to community resources/services and ongoing collaboration with the IDG and community health providersAdditional Job DescriptionEducation: Masters degree in a social work or social welfare program accredited by the Council on Social Work Education. Credentials: MSW licensure by the South Carolina Board of Social Work Examiners.
