Responsibilities include ongoing assessment data collection contributing to individualized care plans and supporting safe age-appropriate culturally relevant care. The LPN evaluates resident responses recommends plan-of-care revisions and collaborates with residents families nursing staff physicians and interdisciplinary team members. This role requires maintaining clinical competency participating in staff orientation/mentoring and following all LTC policies and infection-control standards.
