And we believe equipping you with support and development opportunities is a vital part of delivering an exceptional employment experience.Find a fulfilling career and make a difference with UnityPoint Health.ResponsibilitiesTreatment planning discharge planning and referrals:Develop and maintain the patients treatment plan throughout the patients course of treatment with input from the care teamKeep patient family or others informed as to the patients progress throughout treatment as the patient desires.Complete safety planningComplete discharge planningMay lead psychoeducational groupsCoordinate patients social determinants of care needsPractice de-escalation techniquesProvide patient information and make referralsUnderstand Medicaid and other governmental benefit systems and help patients navigate through the public mental health system.Help patients follow through with their health goals.Coordinate medical and behavioral healthcare for patientsAssists patients with activities of daily livingConducts assessments including but not limited to suicide screening and mental health assessments patient rights obtaining releases of information advanced directives etc.May completes patient rounding in order to maintain a safe environmentProvides culturally and linguistically appropriate servicesMay transport patientsDocumentation and Administrative Functions:Maintains accurate concise and current clinical documentation.Collaboration and CoordinationParticipate in all required trainings and educational activitiesParticipate in department level meetings and huddlesBe responsive to patients in applicable communication modeQualificationsEducation: Bachelors degree required. Mental Health related field prefered.Experience: Previous healthcare & behavioral healthcare experience preferred.License(s)/Certification(s): Drivers license if in an outpatient setting. May require class D Drivers license within 6 months of employment for outpatient setting. May be required to obtain BLS within six months of hire for certain sites.Other: Use of usual and customary equipment used to perform essential functions of the position.
