This role is essential to our mission providing therapy crisis support and service coordination for mental health clients in a variety of settings. As a Intake Coordinator with Carolina Outreach you will be managing and coordinating incoming referrals to ensure timely and efficient access to services.Location: Fayetteville NCCompensation: $20.57 - $23.35Build Your Purpose.Carolina Outreach an ncgCARE partner is a leading provider of community-based behavioral health services in North Carolina. Our comprehensive range of services include school-based therapy outpatient treatment psychiatric evaluations community support and substance use services.Position Description:Receive review and process incoming referrals from various sources ensuring all required information is complete and accurate.Communicate promptly with referral sources clients and families to gather intake information and explain the admission process.Coordinate and schedule intake appointments assessments and evaluations with appropriate clinicians and programs.Assign clients to appropriate providers or programs based on clinical needs location and service availability.Maintain accurate and timely documentation of referral and intake activity in the electronic health record (EHR) and referral tracking systems.Collaborate with clinical and administrative teams to ensure efficient handoff and continuity of care from intake to service delivery.Track referral outcomes monitor pending cases and follow up to ensure timely resolution.The above statements describe the general nature of work being performed and are not an exhaustive list of all responsibilities.Qualifications Bachelors degree in a human services or related field required.Minimum of 2 years of experience working with the mental health population required.Associate professional status required.Qualified professional status preferredGrow with Us.Come be part of an inclusive culture that values excellence innovation and discovery.
