Internal Medicine physician job in Maryland :

OVERVIEW

Our suburban community enriching neighborhood health centerprovides easy access to care for senior patients healthcare needs, with a partnershipto Medicare Advantage plans that fully cover primary care.Each physician at this clinic would maintain a smaller patientpanel size while collaborating with the best outcomes-oriented specialists &hospitals. Each Primary Care Physician (PCP) leads a team consisting of APNspharmacists, care associates, MAs behavioral health support, quality-basedcoders & referral coordinators. This model is positioned to provide conciergeexperience with coordinated care supported by using analytics and tools. Thisrobust support allows you to see fewer patients, therfore spending more timecaring for patients.

ROLE

Outpatient Primary Care for Patients over age 65with Minimal Call of 1:8

Schedule: Mon-Friday -8a-5p

Average 11-14 patients per day

Evaluates and treats center patients inaccordance with standards of care

Assists in the coordination of patient services,including but not limited to specialty referrals when appropriate, hospital andSNF coordination, durable medical equipment & home health care

Acts as an active participant and key source ofmedical expertise with the care team through daily huddles

Examine/analyze patient records, includingallergies, medications, immunization status & detailed medical histories(past medical, surgical, family & social history)

Conduct physical examinations, document findings& diagnose medical conditions using patient history, examination results& diagnostic tests

Administer, order, interpret & act ondiagnostic tests, such as blood work & imaging

Perform preventive & therapeutic procedures,including immunizations, injections, suturing, wound care & infection management.

Develop and implement individualized care plans,providing instruction & counseling to patients.

Manage & monitor chronic diseases such asdiabetes, hypertension, asthma & obesity, ensuring long-term healthoutcomes.

Meets with Lead Physician about quality of care,review of outcome data, and medical records issues

Prescribe medications within state guidelinesand clinic regulations & record patient progress.

Review medication regimens for potentialinteractions, patient safety & adherence to prescribed therapies.

Provide general health education on proper diet,family planning, emotional well-being, & health maintenance.

Offer in-house specialty consultations andservices (e.g., psychiatric care) as appropriate to scope & training.

Refer patients to specialists & collaborateon treatment plans as needed.

Ensure timely and compliant completion ofmedical charts

Engage in community health initiatives such ashealth fairs or patient education workshops to & population health effortsto identify trends + implement targeted interventions for high-riskpopulations.

PACKAGE

Market salary + Bonus ~25% of Salary; Total Year1 will reach $315k or more

Active license to practice medicine in Maryland

Current DEA certificate and TB screening/vaccination

Board Certified or Board Eligible in Internal Medicine orGeriatrics Fellowship Training

Prefer Experience in outpatient/ambulatory healthcaresettings, including residency training; Prefer knowledge of HEDIS, STARS, CAHPSand or HOS

Board Certified or Board Eligible Medical Doctor with or w/o US residency with current US work visa.
