Will follow and direct evidenced based practices as established within the Primary Care service line. Collaborates and coordinates the individual treatment plan with the clinical team as necessary to assure an appropriate continuum of care.Minimum Education Requirement : Graduate of Accredited Medical School. Board Eligible or Board Certification in Internal Medicine or Family Medicine.Minimum Experience Requirement : Must have completed a family medicine or internal medicine residency. Previous experience in group practice or hospital setting with broad scope of family practice or internal medicine experience preferred.Minimum License Requirement : Current Federal Drug Enforcement Agency Certificate.
