What You'll Do: Round at Skilled Nursing Facilities: Manage chronic and acute conditions for patients in designated Orange County nursing homes. Reduce Hospitalizations: Stabilize patients on-site and prevent unnecessary readmissions. Lead Goals-of-Care Discussions: Partner with patients and families to ensure treatment aligns with their values. Drive Quality & Compliance: Close care gaps, support value-based initiatives, and maintain excellent documentation. Collaborate & Lead: Work closely with NPs, nurses, and care teams to deliver seamless, patient-centered care.
