Job DescriptionJob DescriptionPosition SummaryAs a member of the Care Team, the LCSW will provide support and mental health services to patient population asrequested or initiated by provider/patient. LCSW will provide community resources and coordinate referrals asindicated.Principal Duties and Responsibilities1, Provide on demand integrated care consults to a diverse patient population in a fast-paced health center setting incollaboration with medical care team. Population can include pediatrics, family practice, geriatrics and/or prenatalpatients. Consults provided from a biopsychosocial-spiritual framework. Services include screenings, support, briefinterventions, and referrals as deemed appropriate.2. Provide psychoeducational information as appropriate for patients and/or family members or accompanyingcaregivers3. Provide traditional counseling services and support to patients and family members.4. Develop and maintain familiarity with related psychopharmacology5. Collaborate alongside behavioral health team to ensure holistic health care and support of medical team.6. Provide training to staff as needed7. Remain current and knowledgeable about local and regional resources for mental health and substance abusetreatment, developing collaborative relationships where possible8. Participate in outreach educational activities in the community as needed9. Maintain a regular schedule at assigned clinic in collaboration with treatment team10. Participate in quality improvement projects as assigned11. Participate in all meetings and trainings as assignedCompliance and documentation1. Thoroughly, accurately, and promptly document all service rendered in the electronic medical record forevery patient.2. Be knowledgeable of and in compliance with CLIA, OSHA, HIPAA and JCAHO regulations for safety,infection control, equipment operation, confidentiality, and other applicable areas (e.g., patient identification andstandard precautions).3. Participate in proficiency testing as required by NOELA or departmental policy to ensure that proper standards ofcare are being maintained.Required Skills or Abilities*1. Able to work as a member of a multidisciplinary care team2. Good physical stamina and an ability to be standing most of the day.3. Good interpersonal skills are a necessity, including an ability to work well with the variety of ages, cultures, andtemperaments represented among NOELA staff and patients, treating others with kindness and professionalism inall they do.4. Commitment to demonstrating personal integrity through punctuality, honesty, an ability to follow instructions,proper attention to detail in all work matters, and a willingness learn from others.5. An ability to work independently, take initiative, and set priorities in accordance with the needs and mission ofthe clinic.6. Flexibility to adapt to changing or stressful conditions, including unanticipated changes to working schedules orlocations.7. Adequate written and verbal communication skills for communicating coherently and professionally withpatients and co-workers.8. Conscientious of departmental and organizational policies and procedures, and able to embrace and personifythe mission of NOELA.Required Knowledge, Experience, or Licensure/RegistrationMaster's Degree in Licensed Clinical Social WorkCurrent LCSW Louisiana licensureCurrent CPR CertificationHIPPA ComplianceBilingual ability preferredExperience in integrated care setting preferredJob Posted by ApplicantPro
