Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team on a full-time basis. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient care. As an LPN at Bradford Hills, you will have the opportunity to work closely with our interdisciplinary team to develop and implement individualized care plans, conduct assessments, administer medications, and provide supportive care to our residents.
Responsibilities:
Provide direct patient care, including vital signs, wound care, and medication administration
Conduct initial and ongoing assessments of patients' physical and psychological needs
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Administer medications and treatments as prescribed by physicians
Document patient information accurately and thoroughly in accordance with facility policies
Collaborate with other healthcare professionals to ensure seamless and efficient care delivery
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements:
Current LPN license in the state of Pennsylvania
Basic Life Support (BLS) certification
Ability to work in a fast-paced environment with flexibility and adaptability
Strong communication and interpersonal skills
Ability to lift, push, and pull patients as needed
