Job Summary:
Join the Bradford Hills Nursing & Rehabilitation Center team and play a vital role in the lives of our patients as an Evening Shift Licensed Practical Nurse (LPN)! Located in the beautiful mountains of Northeast Pennsylvania, our state-of-the-art center is dedicated to providing high-quality care and personalized attention to each of our residents. As an LPN, you will work closely with our interdisciplinary team to deliver compassionate and innovative care that exceeds our residents' expectations.
Responsibilities:
Provide direct patient care to our residents, including administering medications, performing wound care, and assisting with activities of daily living
Conduct thorough assessments, develop care plans, and implement interventions to promote optimal health and well-being
Collaborate with the healthcare team to ensure seamless transitions and coordinate care with other healthcare providers
Maintain accurate and detailed records of patient care, medication administration, and treatment plans
Participate in in-service training and educational programs to enhance your clinical skills and stay up-to-date on the latest industry developments
