Job DescriptionJob DescriptionMVA is looking for a LICSW/LPC that completes intake sessions. Develops individual care plans for behavioral health patients. Provides clinical evaluation, intervention, and planning for patients to improve patients/families functioning, overall patient wellness, and medical adherence. Provides risk assessment and intervention, with emphasis on reducing harm. Facilitates crisis intervention and emergencies services when necessary. Provides recommendations and coordination of external psychiatric and/or social resources based on assessment and collaboration with the patient/family. Acts as liaison between patient/family, behavioral health providers and medical health providers to ensure clear communication amongst the care team. Provides documents and reports for referrals. Ensures adequate documentation of patient encounters for billing purposes and to ensure continuum of care. Provides case management around psychosocial/behavioral health needs.
